Dear Dr Michael Wright, RACGP President, Dr Siân Goodson, Chair of the RACGP Board of Directors, and the members of the Board of the RACGP, For twenty-one months, we have watched the Israeli state continue to inflict unimaginable suffering, injury and death on the besieged population of the Gaza Strip. The official death toll of the assault on Gaza has crossed 60,000, 18,000 of them children. This is widely recognised to be a gross – even misleading – underestimate. The Lancet in July 2024 reported that even its conservative estimate of 186,000 dead would represent the killing of 7.9 per cent of Gaza’s pre-October 2023 population — this is an extermination. There is a growing consensus amongst Palestinian, Israeli and international NGO’s, United Nations bodies, and genocide scholars and historians that Israel’s actions in the Gaza Strip constitute the crime of genocide. In that time, we have witnessed a systemic failure across the entire ecology of Australian medical colleges to “mount [any] meaningful protest” against the horrors besetting the Palestinian people in Gaza. We, both members of the RACGP and affiliated GP’s, urgently demand the following three clear actions from the College: An unequivocal and substantive condemnation of the Gaza genocide that identifies Israel as the perpetrator of the constitutive acts. We are not legal scholars and are not required to adjudicate on the legal merits of the charge of genocide. However, there are phenomena that are self-evident in their incompatibility with the principles of international humanitarian law, human rights, medical neutrality, and medical ethics that the RACGP is dutybound to call out. Previous RACGP statements have been sterile, devoid of commitment, and have been degrading in their obvious indifference to the mass liquidation of Palestinian lifeworlds inside the Gaza Strip. A declaration of solidarity with our Palestinian healthcare worker colleagues who continue to work tirelessly and heroically under overtly exterminatory conditions. Honour them, commend them, call loudly and publicly for their protection. They are our colleagues, and they embody the principles of medical ethics and professionalism that the RACGP is at risk of turning its back on. A commitment to lobby the Albanese government to use every means available to it to bring an urgent end to the war crimes in Gaza including diplomatic and economic sanctions, arms embargoes, and multilateral action within the United Nations. Background It is hard to capture the breadth and ferocity of the assault on Gaza, with every conceivable aspect of Palestinian life decimated by the Israeli military campaign. As healthcare workers, our ethics compel us to highlight the obvious health catastrophe unfolding in Gaza. We must heed the harrowing testimonies from dozens of our colleagues like Dr Bushra Othman, Dr Mohammed Mustafa, Dr Feroze Sidhwa, Dr Nick Maynard, Dr Tanya Hajj Hassan and Dr Victoria Rose describing patterns of injuries amongst the Palestinian population that beggar belief – including the deliberate targeting of children, the use of civilians for “target practice”, and widespread use of sexual and gender-based violence. We decry the precise campaign of mass starvation engineered by the Israeli government inside Gaza, beginning with the systematic destruction of Gaza’s agricultural land, water wells, olive groves, greenhouses, fisheries and livestock, creating a near-absolute dependence on humanitarian aid. The blockade on the entry of all food, medicine, water and baby formula, has pushed the population in Gaza to a health and nutritional tipping point. We are seeing an alarming spike in the number of deaths by starvation disproportionately affecting children under the age of five. Even infant formula has been confiscated by Israeli forces from medical workers entering Gaza to volunteer at the hospitals. The IPC has issued an alert that the “worst-case scenario of famine” is unfolding in the Gaza Strip, with 76 per cent of the population facing emergency or catastrophic level food insecurity, and reports that one third of the population goes for “days” at a time without eating, placing thousands at immense risk of refeeding syndrome. Meanwhile, more than 1,000 civilians have been killed at the few aid distribution points that have been turned by the Israeli military into “kill zones”, where soldiers fire at will at starving and dehydrated aid-seekers. MSF has called this travesty of aid distribution a system of “orchestrated killing”. Amnesty International this month has released a report evidencing its position that Israel continues to use starvation as a tactic of genocide. Gaza’s entire healthcare sector has been eviscerated. Systematic attacks on healthcare infrastructure have left 94 per cent of Gaza’s hospitals and clinics damaged, destroyed or besieged. At least 1,580 healthcare workers have been killed in Gaza, and as at September last year, Healthcare Workers Watch had documented at least 264 cases of unlawful detention of healthcare workers, with widespread reports of severe physical and sexual abuse, torture, and even murder while in detention. A few warrant special mention: Dr Adnan al-Bursh, former head of orthopaedics at al-Shifa Hospital, who was kidnapped in December 2023, and died in Israeli custody four months later. It has been reported that he was sexually tortured to death. Dr Hussam Abu Safiya, paediatrician and director of Kamal Adwan Hospital, who was kidnapped during a month-long siege of the hospital in December 2024 and remains in Israeli custody without charge, reportedly critically unwell and enduring conditions of torture, solitary confinement and medical neglect. The fifteen ambulance workers from the Palestinian Red Crescent Society and the Palestinian Civil Defence teams who were executed after Israeli forces opened fire on them in the Rafah Governorate, then buried their bodies and their vehicles in a nearby sandy grave. Footage from the incident found about a fortnight later revealed that they were travelling as a clearly marked convoy of first responders in uniform, with lights and sirens. Dr Ala’a al-Jarrar, a paediatrician, whose husband was slaughtered along with nine of their children in their sleep in May 2025, targeted by a drone while she worked an overnight shift at Nasser Medical Complex in Khan Younis. Precedent and Rationale We do not consider this a partisan issue and forcefully reject the language of “political neutrality” and “diversity in opinion” as irrelevant and inappropriate responses to the crimes taking place in full view. It sets a frankly dangerous precedent for the RACGP to entertain that there could be a diversity in opinion about a genocide. The mass trauma, biological injury and moral harm caused by the events in the Gaza Strip are truly staggering, and will take generations to heal. The RACGP has a professional and a moral duty that it has failed to uphold in its continued silence on the world-historic atrocity in Gaza. By staying silent, it has undermined our medical ethics and consequently contributed to the ongoing impunity that allows the Israeli government to continue its crimes in Gaza without accountability. Our foundational ethical principle is to do no harm. In its silence, the RACGP is doing harm. We wish to draw Board members’ attentions to the statement made by the New Zealand College of Public Health Medicine that plainly condemns attacks on civilians and calls for action to prevent genocide in Gaza, or the joint statement from peak emergency medicine and nursing bodies in Australia and Aotearoa New Zealand calling for a ceasefire and for the protection of civilians and healthcare workers. Internationally, we can point to other joint public statements that are principled in their rejection of genocide and the targeting of healthcare workers, or the fantastic and ongoing interventions by the British Medical Association. There is precedent for the RACGP to do much, much more. Deadline for Action We call for the RACGP to action these three demands by 8 October 2025 – marking the two-year anniversary of the genocide in Gaza. If after two years of these open horrors, the RACGP and board members still refuse to take this minimum moral and professional stand, this will inevitably lead to a further corrosion of our trust and confidence in the culture of the RACGP and the character of its board members. It will invite a further response from us as concerned College members and non-affiliated GP’s who stand opposed to genocide, including considering withdrawing our financial membership in the RACGP.
